OUTREACH SERVICES
The staff at the Council on Aging are available to provide information, referrals and support to Provincetown seniors and their families. Our Outreach Coordinator, Chris Hottle, social worker, will talk with you in your home, at the office or by phone to help with any concerns you may have. All services are free and confidential to Provincetown seniors and their families.

If you are experiencing difficulties living alone, are having problems of any kind at home, are considering a nursing home, need supportive counseling, are concerned about neglect or abuse of a senior in the community or just don't know where to turn for assistance, please call us for an appointment 508.487.7080.

SOCIAL SERVICES
Our Social Worker, Patrick Manning, offers private appointments at the Senior Center to assist you with applications for services or forms. Call for an appointment 508.487.7080 for help with emergency financial needs, disability questions, housing,income, Food Stamps, the Perscription Advantage or MassHealth.

THE SHINE PROGRAM
"SHINE" stands for "Serving Health Insurance Needs of the Elderly." Our SHINE Counselor, Dennis Wojtalak, is available by appointment to assist you with health benefits coverage or fuel assistance needs. If you have questions about Medicare, Medigap, Medex or Fuel Assistance, call for an appointment 508.487.7080. Home appointments are available for physically challenged seniors.
