Outreach Services


Outreach assistance connects you to community and regional services and programs. We also provide home and office visits for personal supportive services such as those listed below. Contact the Council on Aging at 508.487.7080 or email Council on Aging

  • Outreach HomeVisits for Homebound Elders
  • Home Care Assessment & Referrals
  • Protective Services (abuse/neglect of an elder) and Elders-at-Risk Assessment and Referrals
  • Friendly Visitor Program for Homebound Elders
  • Social Services Assistance (helps with financial needs, housing, Food Stamps and MassHealth applications)
  • Fuel Assistance
  • Health Care Insurance Information (guides you to various insurance alternatives and assists you with applications and forms)
  • Community Resource Assistance Program (coordinates home chores for seniors)
  • Medical Equipment Loan Closet (loans wheel chairs and other items)
  • Police/Senior safety support services (Telephone Reassurance Program, lock box, coordination of services with TRIAD Liason Coordinator)
  • TRIAD/S.A.L.T Council (Seniors And Law enforcement Together), whose mission is to keep seniors safe in the community
  • USDA Food Distribution

The support groups listed below are available to all residents. Meetings are held at the Grace Gouveia Building located at 26 Alden Street. Contact the Council on Aging at 508.487.7080 or email Council on Aging for meeting schedules and answers to your questions. Confidentiality respected.

Nutrition Programs

The Council on Aging provides a variety of nutritional programs in a social setting at the Cafe Gouveia and hosts the Meals On Wheels program. Contact the Council on Aging at 508.487.7080

  • Meals On Wheels (Elder Services of Cape Cod & Islands-Age 62 to participate)
  • Congragate Lunch (Wednesday at Noon)
  • "Back door" Pantry (Emergency food pantry)
